RESUMO A retenção da membrana de Descemet (MD) do olho receptor é uma rara complicação da ceratoplastia penetrante (CPP). São descritos dois casos de retenção acidental da MD e de lamelas posteriores do estroma corneano em CPP, identificadas no pós-operatório como membranas retrocorneanas (MRC). No primeiro paciente ocorreu o contato entre a MRC e a superfície endotelial do enxerto, ocasionando a sua descompensação. O exame histológico confirmou a presença de dupla membrana de Descemet. No segundo paciente, não houve o contato entre a membrana e o enxerto, que manteve-se transparente. Contudo, houve progressiva opacifícação da MRC, sendo necessária sua abertura com Nd: YAG laser para obtenção de melhora na acuidade visual. A biomicroscopia ultra-sônica, pela primeira vez realizada nesses casos, permitiu identificar em uma maior extensão as duas camadas que constituem a MRC: as lamelas estromais posteriores e a MD. São feitas observações sobre a etiopatogenia, formas de prevenção e tratamento dessa complicação cirúrgica que, caso não seja identificada e tratada adequadamente, pode levar à falência do enxerto. Trata-se da primeira descrição dessa complicação cirúrgica na literatura nacional.
SUMMARY Retention of host's Descemet’s membrane (DM) is an unusual complication in penetrating keratoplasty. Two cases of accidental retention of Descemet's membrane and posterior lamellaes of corneal stroma in penetrating keratoplasties, identified postoperatively as a retrocorneal membrane (RCM) are reported. In the first patient there was contact between the RCM and the donor's endothelial surface, ensuing graft failure. In this case, histologic examination confirmed the duplicated Descemet's membrane. In the second patient, the membrane has never in contact with the graft, which remained clear. However, the RCM became progressively opaque and the Nd: YAG laser was required to create a opening and improve the visual acuity. The ultrasonic biomicroscopy, used for the first time in such cases, identified the two layers of RCM: posterior stromal lamellaes and DM. Observations about the etiopatogeny, prevention and treatment of this surgical complication are made, which may lead to graft failure if not properly identified and treated. To the best of our knowledge, this is the first description of this surgical complication in the brazilian literature.
